Here’s how to go about getting a replacement SS card… step-by-step:
Duplicate Social Security Card-
- If you need to order a duplicate social security card- go to: https://www.ssa.gov/forms/ and you can find any form you need related to social security.
- Search for SS-5 which is titled – Application for a Social Security Card. This is the form used to get a replacement card. Follow the instructions to complete the form properly.
- The client must take this form to the local SSA office with an NC ID or a Certified Medical Record as a form of ID and show their ID to the clerk to have them process the form to get a duplicate SS card.
- Call the Social Security Office at (888) 383-1598 and make an appointment. You can no longer just walk into Social Security and get assistance without an appointment!
- The Social Security Office is located at 2201 Coronation Blvd off of Independence Blvd. Click the link above for a map. Bus #17 runs out there every 60 minutes. Get off at Crownpoint Executive Dr and Sardis Rd. Then walk a block down Sardis Rd N towards Monroe Rd until you get to Coronation Blvd
